FEE AGREEMENT
The undersigned hereby requests and authorizes Wallace D Cegavske,
Attorney at Law hereinafter called "Attorney," to represent me as
"Client" in the
following matter:
1. Type of Matter. Limited representation to advise and assist me in
representing myself to obtain a divorce from my current spouse.
2. Potential Defendants. My spouse.
3. Terms.
Client agrees to keep contact with and cooperate with Attorney in providing
requested information and to at all times provide Attorney with Client's
current address and telephone number.
Client agrees that Attorney's representation is limited to advising client
and preparing appropriate divorce documents. Client understands that without
further agreement, Attorney will not deal directly with, or negotiate with,
Client's spouse or anyone on behalf of Client's spouse.
Client retains the right to terminate Attorney's services at any time by
delivery of a signed letter notifying Attorney of the termination of the
Attorney/Client relationship.
Attorney retains the right to withdraw as Attorney for Client upon breakdown
of the relationship; upon nonpayment of attorney fees or costs due; upon
failure of Client to keep Attorney notified of current address and telephone
number; failure to keep contact with Attorney and failure cooperate with
Attorney.
4. Client representations.
Client understands that Client is entitled to absolute confidentiality, but
in order to achieve the cost savings of participating in workshops and
communicating by FAX, Telephone and email, Client waives the right of
confidentiality to the extent necessary to allow Attorney:
a. To answer questions Client raises during workshops, and
b. To communicate with Client by email, cell phone and FAX where the
necessary information to do so has been provided by Client.
Client understands and agrees that Attorney reserves the right to refund
Client's advance payment and thereupon refuse to draft and or file documents
in the case.
Client understands that Attorney's representation is limited as set forth
above and that representation by the Attorney other than as set forth above
will require a separate agreement.
Client realizes that Attorney does not warrant or predict developments in or
final results of the proposed representation. Attorney does assure Client of
Attorney's commitment to afford Client conscientious and faithful service
within the scope of this agreement.
Client acknowledges that Attorney will be relying on information furnished
by the client and that Attorney assumes no duty to verify the information
furnished. By Client's signature to this form, Client is releasing Attorney
from all claims for damages arising from incorrect information that may be
furnished to Attorney by Client.
5. Payment Election.
[ ] I have enclosed my check for the amount of the fees and costs.
6. Amendment. Attorney and Client may alter or amend this Agreement by
signing a written amendment to this Agreement.
7. Total Agreement. Client has read this Agreement and agrees to its terms
and conditions. There are no verbal agreements between Client and Attorney
modifying or expanding the terms of this Agreement.
